First Year of Fellowship

Renal Consultation at NewYork-Presbyterian Hospital (two months):
Fellows gain experience in the evaluation and treatment of hospitalized patients in medical, surgical and obstetrical services with electrolyte and acid-base problems, acute renal failure, acute/rapidly progressive glomerulonephritis, nephrotic syndrome and high blood pressure. Additionally, fellows perform and interpret kidney biopsies. This rotation includes managing renal failure and complex fluid, electrolytes and acid-base problems in critically ill patients admitted in the medical intensive care unit (ICU), cardiac ICU, Burns ICU, and neurology ICU. This rotation also includes managing renal failure and electrolytes and acid-base problems in patients admitted at the Hospital for Special Surgery.

Onco-Nephrology at Memorial Sloan-Kettering Cancer Center (three months):
Memorial Sloan-Kettering Cancer Center is a 480-bed institution specializing in cancer and allied diseases. Fellows gain experience in onco-nephrology. This rotation emphasizes problems of renal drug toxicity related to chemotherapeutic agents, evaluation and treatment electrolyte and acid base disturbances, metabolic aspects of hyperalimentation, and renal disease related to bone marrow transplantation. The range of metabolic, mechanical (urologic) and functional disorders that the fellows are exposed to in this rotation is extremely unique.

Weill Cornell Medicine Nephrology Hypertension Rotations

Transplantation Medicine at NewYork-Presbyterian Hospital (one month):
The transplant program performs more than 250 kidney transplants every year. It is a multidisciplinary service and the fellow and the renal attending make daily bedside and teaching rounds on transplant patients. They also work with the transplant surgeon and the surgical resident and ancillary personnel such as pharmacists and social workers. Fellows gain experience in managing all aspects of kidney transplantation and pancreas transplantation. The fellows observe and manage about 15-20 new transplant recipients during each month of their rotation, with direct responsibility for evaluation and management of every aspect of the care of transplant patients. This includes evaluating patients pre-transplant and clearing them for transplant surgery, management of fluid and electrolytes in the immediate post-transplant period, immunosuppressive medications and dialysis, if needed. In addition, the fellow evaluates patients for post-operative complications including surgical complications, cardiovascular and metabolic problems. The fellows also perform and interpret transplant renal biopsies.

Renal Medicine & Hypertension at NewYork-Presbyterian Hospital (two months):
The Renal Medicine & Hypertension rotation is a combined consultative, nephrology and hypertension, teaching rotation. Patients on the renal medicine service include patients with either acute or chronic renal problems, excluding newly transplant patients. The renal fellow is responsible for teaching medical housestaff, and is encouraged to prepare short lectures at attending rounds, covering basic renal problems. During this rotation, fellows are also on-call for inpatient hypertension consults at NewYork-Presbyterian/Weill Cornell Medicine. The fellow has the opportunity to evaluate and manage patients with primary and secondary hypertension, obstetric hypertension, hypertension associated with aortic dissection, and all hypertensive urgencies and emergencies.

Dialysis and Apheresis at NewYork-Presbyterian Hospital (two months):
The acute hemodialysis unit has ten acute dialysis stations as well apheresis stations. During this rotation fellows gain experience in all aspects of inpatient dialysis (excluding intensive care units) as well as inpatient apheresis for renal and non-renal conditions.

Critical Care Nephrology at NewYork-Presbyterian Hospital (one month):
This rotation includes managing patients with cardio-renal syndrome who are admitted in the ICU as well as kidney-related issues in surgical patients. Rotations in the ICU will include learning about all aspects of continuous renal replacement therapy.

Elective (one month):

Fellows have the opportunity to do one month of an elective rotation. This includes, but is not limited to, rotations at the immunogenetics and tissue typing laboratory, renal pathology, Hypertension, Outpatient Dialysis (home, PD, nocturnal), Nuclear Medicine and Radiology.

Research/Elective (six months):
Our program is committed to scholarship, translating the best evidence to clinical care and creating new knowledge. Accordingly, six months (including one month elective, as above, if required) is reserved for scholarly activities in the area of basic research, translational research or clinical research. The mentored research activities ensure full realization of the wide array of research activities available for full participation by fellows. The investigators in the Division of Nephrology and Hypertension have been continuously funded for decades by the National Institutes of Health, and have ongoing collaboration with researchers at the neighboring Rockefeller University and Memorial Sloan Kettering Cancer Center. Over the years, our fellows have been highly productive in terms of presentations at the National and International meetings as well as publishing in high impact journals.

Outpatient Clinics

Outpatient Continuity Clinic at NewYork-Presbyterian Hospital:
Fellows are assigned continuity clinic one afternoon per week starting in year one for a total of 24 months excluding vacation. Each fellow will have one attending precepting for the entire year. Fellows are involved in the management of all aspects of outpatient care for patients with kidney diseases.

Outpatient Transplant Medicine Clinic at NewYork-Presbyterian Hospital:
During their inpatient transplant rotation (three months), fellows continue the outpatient management of kidney transplant recipients who received their transplant during the fellow's inpatient rotation. The fellows are involved in all aspects of medical care of the transplant recipient, including evaluation of allograft function, immunosuppressive drug doses, and management of infection-related issues.

Outpatient Dialysis Clinic at NewYork-Presbyterian Hospital, First Year of Fellowship:
During their inpatient dialysis and renal medicine rotations, which will last five months, for one afternoon each week, fellows participate in multi-disciplinary conferences in the PD unit with Nutrition, Social Work and Nursing staff covering all aspects of a patient's case. The Rogosin Kidney Center's Peritoneal Dialysis Unit at NewYork-Presbyterian Hospital treats more than 40 patients managed by CAPD and CCPD. The fellow evaluates patients during their routine care and follows them when they develop complications of PD. The fellow participates in the training of new peritoneal dialysis patients, as well as help patients select the appropriate PD modalities based on PET test results. The fellows are involved in the ongoing management of PD patients, including assessment of dialysis adequacy, anemia and iron management, acid-base and electrolyte issues, metabolic bone disease and management of hospitalized PD patients.

Outpatient Dialysis Clinic at NewYork-Presbyterian Hospital, Second Year of Fellowship:
During their research and elective rotations, which will last seven months, fellows are provided with an opportunity to rotate, one afternoon a week, in the Rogosin Kidney Center's Hemodialysis Unit at NewYork-Presbyterian Hospital. The unit has 37 hemodialysis stations catering to patients in four shifts, six days a week. Fellows participate in multi-disciplinary conferences in the HD unit with Nutrition, Social Work and Nursing staff covering all aspects of a patient's case. The fellows are involved in the ongoing management of HD patients, including assessment of dialysis adequacy, anemia and iron management, acid-base and electrolyte issues, metabolic bone disease and management of hospitalized HD patients.
